Copying and Editing Student Learning Feedback Forms (SLFFs) in EvaluationKit

How to copy and edit Student Learning Feedback Forms (SLFFs) to update them for a new semester.

Body

As you teach a course multiple times, you may find that the course objectives, assignments given, or resources used change as you refine your approach. This will mean that the Student Learning Feedback Form needs to be altered to reflect what you have taught in the current semester. You can edit your SLFFs in EvaluationKit to ensure that your students will be asked for feedback about the current iteration of your course at the end of the semester.

Warning: If you change your previously-created SLFF in EvaluationKit, it will change all instances of the form, including its use in previous semesters, making your results unusable. Therefore, to change an SLFF, you must copy your previous SLFF and then edit the copy.

When you copy your previously-used SLFF, it will copy exactly the number of questions you had in the original form, so if you deleted “extra” questions (ie, you only had 4 assignments originally so you deleted the question for Assignment #5), they will not appear on the copy either. You can re-add these questions from the master SLFF template if needed in the new SLFF.

How to copy a Student Learning Feedback Form (SLFF)

  1. Go to the EvaluationKit website and log in with your credentials
  2. At the top of the screen, click Custom Questions, then Custom Question Surveys
  3. To the right of the SLFF you want to edit, click the Copy icon
  4. You will have the option to give your copied survey a New Survey Name. Title your SLFF using the following convention: Course Prefix and Number – Course Name – Semester [Ex: PHYS112 – Intro to Astronomy – Spring 2022]
  5. Choose the second radio button: Create a new instance of this survey
  6. Click Copy

How to edit a Student Learning Feedback Form (SLFF)

  1. To edit your previously entered custom text, to the right of the question, click the pencil icon
  2. Edit your custom text in the textbox
  3. Click Update Question Text
  4. Repeat Steps 1, 2, and 3 for all questions desired
  5. At the top of the page, click Save

How to re-add questions from the original SLFF template

  1. At the top of the page from the Select Question Type dropdown menu, select LO Item Bank Question
  2. Click + Add
  3. From the dropdown menu, choose SLFF Template
  4. Click Get Questions
  5. Above the questions you want to re-import, click the checkbox
  6. At the top of the page, click Add selected
  7. Proceed to edit the custom text in these re-imported questions following the instructions in the previous section

Once you have created your Student Learning Feedback Form (SLFF) in EvaluationKit, you must attach it to the correct course(s) for it to be deployed to the students at course evaluation time.
HWS will use EvaluationKit to allow faculty to receive feedback from students based on course-specific learning objectives. Faculty will be able to customize the Student Learning Feedback Form (SLFF) template to reflect the goals, concepts, and skills taught in each of their classes and then deploy that form to their students using EvaluationKit.
