Attaching a Student Learning Feedback Form (SLFF) to a Course in EvaluationKit

Once you have created your Student Learning Feedback Form (SLFF) in EvaluationKit, you must attach it to the correct course(s) in the current semester’s project for it to be deployed to the students at the end of the semester. SLFFs can be attached to multiple courses, so if you are teaching two sections of the same course, or a cross-listed course, you only need to create one SLFF and attach it to both.

Please note that this process must be completed individually for each SLFF you wish to use for this semester’s student feedback. You will be selecting your SLFFs one at a time and attaching them to one course (or several sections of one course) at a time.

Please also note that if you combined your sections or cross-listed courses in Canvas, you will only see the “parent” section or course in EvaluationKit. The SLFF will go to both courses or sections.

How to attach your SLFFs to your classes

  1. Go to the EvaluationKit website and log in with your HWS credentials
  2. At the top of the screen, click Custom Questions, then Custom Question Projects
  3. To the right of the current semester's project, click the + icon
  4. Click Add Custom Question Survey
  5. Select the SLFF you want to add by clicking the checkmark icon
  6. Click Select Courses
  7. Click the Add Courses button
  8. On the left, click the checkboxes of the courses to which you want to attach the SLFF
  9. Click Add Selected Courses
  10. A message will pop up saying that you successfully attached the SLFF to your course(s)
  11. Repeat steps 4-9 for each of your SLFFs
